Please, which plugin can I use to achieve this feature on WordPress?
Sms or email will be sent out to people depending on what they chose.
I don't know how to explain it perfectly.

Hello @Chieyine,
Try using Gravity Forms with a few addons which are available here on Babiato. It should help you achieve what you are looking to build. For notifications, try using the Select a field notification with selecting the field as the user entered email address.
