there have a lot plugins like that. You need to search "conditional form". Gravity form, AR form, Nexform.
Or you can search "quiz" plugins especially Adviser Quiz plugin ( https://codecanyon.net/item/advisor-quiz/21860992 )

As I said before you can make this with 2 ways. One of them is "Form" I strongly suggest to you "Gravity form" (there have a lot of addons please search it).
Or you can make this with survey/quiz plugin and I was used and recommended this plugin --> "Adviser Quiz"
