problem with mystickyelements wordpress plugin

problem with mystickyelements pro wordpress plugin.
I have unchecked the "Required" option under "Dropdown" and "Consent checkbox" (see screenshot)
but both are appearing as required, means visitor have to forcible select these two options. I have disabled,removed,re-install and re-activate the plugin but all in vain..cache cleared many times, tried different browsers but problem still exist.. any help will be highly appreciated. or is there any way to remove plugin's footprints in database after removing plugin..because when I reinstall after deleting, it automatically restore all settings,text,mobile no, email no automatically (may be from database)
