Finally loaded. That's not a plugin. It's a custom multistep form made with html and javascript.
You have all the code you need to replicate it in the source of the page.

Its seem to be made with microsoft form too. I can be wrong, mais ça semble être le cas.

that "msform" id is actually multi step form and it's also mentioned above in <!----- multistep form ------> or something like that

Search for a multistep form plugin or creator. I didn't use any so I can't recommend you a best one.
