Hello,

I needed some help on how to create a form in WordPress with multiple stage questions.
So the client wants to create a small form, with 5 stages

Example
Q1.Which is your favourite colour
(a) Red (b) Green (c) Black

The user selects (a) Red .. Based on this selection The next Question is displayed

Q2. Which is your Favorite Red Fruit
(a) Apple (b) Tomatoes (c) Strawberry

The user selects (c) Strawberry .. Based on this selection The next Question is displayed

Q3. From Which Location do you like your Strawberries
(a) Europe (b) Africa (c) Asia

The user selects (b) Africa .. Based on this selection The next Question is displayed

similarly, there will be a few more questions and then the Final predefined answer will be displayed, which is basically a Product (The client has 10 products) so based on the previous selections a product will be shown, along with a link.

I tried Forminator Pro Quiz Module, but it does not let me create multiple questions.
I don't know if there is an option like that in Wpforms, i checked and they had a survey thing, but that's not what i want.

Thanks for your patient read.
Appreciate every help

Can't you use Conditional Logic? Most of the WordPress forms has conditional logic. WP Forms, Fluent Forms, Formidable Forms, Gravity Forms, Ninja Forms etc...

I was looking for a form that does the same thing (kinda)
And I can confirm that Piotnet Forms works like a charm for me
You should give it a try
