[Help] Create a table from formidable PRO checkbox inputs

I am looking for a formidable pro user to help me with some issue that I'm facing. I have a dynamic field which is a checkbox. I am pulling data from another form into this dynamic field. For example, I have a form which creates users so the dynamic field is helping pull their data into the current form. I need to allow a user to select a user a trainer from the list of users (from the dynamic field) but the challenge is that I cannot present all the data in tabular form with the checkmark on the left. Here is what I intend to achieve. Any help will come in handy because I have taken days trying to achieve it without success. I have tried to follow their grid classes but I can't quite get it to work. If you have not used formidable forms and don't understand how dynamic fields work this might be a little technical but any pointers are welcome.
