I installed the same addon . And copied the upload folder to root.. still the same

Files are there, but it says 404 when I opened the logo image from site to new tab. Inside file manager they were working fine. File 0644 permission folder 0755

If you still have issues with icons and js, its because you have to manually upload some folders from the addon, that will fix it.
