Have you tried uninstall - reinstalling the plugin ? If still happening after that , then I'll post the one i'm using.

Cheers I am well aware of exactly what is causing this issue though, I have dealt with it myself, basically your sites cached/minified javascript files will still have old Yoast code in it causing conflict and no metabox to appear. You will need to force the old minified file/files to be regenerated and reminified to solve this issue, you can do this by either fully clearing all your caches or by disabling and re enabling minification options - many plugins and themes have options to minify all js files into one, see below for examples.
